Terms of Service

These terms describe use of the Zulqarnain MD PC website and general inquiry process. An accepted written engagement agreement controls the services, scope, fee, and responsibilities for your specific matter.

Service availability and engagement

A website inquiry, phone call, or payment does not by itself establish a physician-patient relationship or an expert engagement. A service begins only after suitability, any conflict review, applicable licensure requirements, and written scope have been addressed and the practice accepts the engagement.

We welcome U.S. and international inquiries. The services available depend on the patient's location, the nature of the request, and applicable professional requirements. A record review does not automatically authorize diagnosis, treatment, prescribing, or ongoing care in any jurisdiction.

Scope of review

Reviews rely on the records and information available. Findings may change when additional information is received. Concierge support and record review complement your established care. Tests, prescriptions, and changes in treatment should be addressed with an appropriately licensed treating clinician.

Medical-legal review is an independent clinical service. Healthcare-law experience does not constitute an offer of legal representation or legal advice. Reports, depositions, testimony, and expedited work require a separate agreed scope. A review does not guarantee a particular clinical, insurance, or legal outcome.

Free introductory service

A limited review and a 15-minute introductory consultation are free. This brief introduction helps discuss your main concerns and the scope of any further services. No payment is required. Comprehensive record review and ongoing concierge care are separate paid services, with scope and fees agreed before proceeding.

Fees and payments

The comprehensive initial concierge engagement is $5,000 and includes the first 15 hours of professional time. Reviews can cover a long history, including 20 years or more, and there is no page-count limit. It covers case and record review, comprehensive consultation, medical questions, and calls to providers for clarification with your authorization. Continued professional time after the initial 15 hours is $300 per hour, billed in 15-minute intervals at $75 per interval, with prior agreement.

There is no page-count limit. Record analysis, consultations, provider calls, and care coordination all count toward professional time. A review requiring more than the included 15 hours may incur additional agreed hourly fees. This is a time-based engagement, not an annual or monthly membership. Independent medical-legal review, reports, and testimony are quoted separately.

The practice confirms scope, time accounting, payment schedule, response and coverage arrangements, and any additional fees in writing before payment. Pay only after the case is accepted and you have an invoice. Verify the recipient or use the hosted payment link supplied by the practice. Do not send card or bank credentials by email, fax, SMS, or website inquiry. This website does not store payment credentials or initiate automatic recurring charges.

Dedicated physician and 24/7 access

Concierge services provide access to one dedicated physician for medical questions, comprehensive review, and primary care support where permitted. You may contact the physician at any time; expected response, coverage, communication channels, and engagement duration are set out in your agreement. Professional time is accounted for within the fee structure above. The service does not replace emergency care or guarantee immediate treatment, hospital admission, or in-person services in every location.

Cancellations and refunds

The written engagement sets any cancellation deadline, deposit, and refund terms before payment. Please request those terms before paying. If the practice cannot accept your case, no service fee is due. If a payment is sent before acceptance or in error, contact the practice promptly so it can be reviewed.

Insurance and other costs

Practice fees do not include outside clinicians, diagnostic testing, medications, hospital care, travel, or other separately incurred costs. Insurance reimbursement is not guaranteed. Confirm your coverage directly with your plan.

Communications

Phone, email, fax, and SMS are for appropriate business communications. Please request the agreed secure method before sending medical records. General email and SMS are not emergency channels and should not contain sensitive clinical details.

SMS is optional. Message types, opt-in choices, frequency, fees, and opt-out instructions are described in our SMS Terms & Consent. The Privacy Policy describes information handling.

Urgent or emergency care

This website is not an emergency service. For emergencies, contact your local emergency services. Do not wait for a website, email, or SMS response.
